Covid-19 Vaccination Web Form

“Good morning,

We heard from NWT residents and other stakeholders that they are not aware of or learn too late when vaccine clinics are being offered in their communities. To address this concern, NTHSSA has set up an online COVID-19 Vaccination Registration web form. By submitting their contact information, residents will receive advance notice of when vaccination clinics are being offered in their communities. Residents needing more information about the vaccine can also put in a request, and a health care professional will reach out to them to provide more information so that they can make an informed choice on whether to be vaccinated. Residents can also register by calling their health centre. Residents will be asked to provide their name, contact information, age bracket and the community they reside in. If they provide an email address, they will receive an automated email confirming receipt of their submission. All personal identifying information is optional, with the exception for vaccine choice and home community. The collected information will enable NTHSSA to facilitate COVID-19 vaccine clinic planning as well as directly contacting residents to ensure they are aware of vaccine clinic availability in their community. The COVID-19 Vaccination Registration web form is available to all communities with the exception of Fort Smith, Hay River, Inuvik and Yellowknife, which already have weekly vaccination clinics in place.
